Travel Nurse Checklist
If you are a first-time traveler, do not panic! We have put together a checklist of basic requirements you would need to get started.
In order to have a complete file the following documents are required both by Clover Health Services and Facility:
Licensure/Certification
- RN License - Copies of all active professional licenses
- All travel nurses are required to have a BCLS card issued by the American Heart Association
- American Heart Association ACLS, PALS according to Specialty
Medical Documents
- TB/PPD – Must have date it was placed, date when it was read, and size of indurations and result (negative or positive)
- Titer results with lab values for the following (Taken within the last 10 years): Rubeola, Rubella, Varicella, Hepatitis C, Hepatitis B and Mumps titers
- Physician statement completed within 12 months prior to your start date
- Documentation of an annual seasonal influenza vaccination
- Respirator fit test documentation
Accounting Documents
- I-9
- W-4
- Picture ID (Drivers License Driver’s License or State ID
- Social Security Card or US Passport
- Permanent Tax Declaration
Others
- Clover Health Services Policy Document
- Facility Specific documents – These vary from time to time according to Facility requirements
- Latest resume
- Professional Reference - contact information for your job references
- Competency Exam with passing score 80%
